I hereby authorize Victorious Life Christian Academy to all of the below:

1. To care for my child during the time he or she is at the facility.

2. To provide medical attention and care for my child if needed and VLCA is not able to reach me.

3. I agree to pay all registration (NON-REFUNDABLE) and enrollment fees instituted by the facility.

I understand that Victorious Life Christian Academy CANNOT release my child to anyone without my signature or approval. The persons listed below have my permission to pick up my child. Each person must show valid Id before a child is released.

As the parent agency representative or legal guardian, I hereby give consent to Victorious Life Christian Academy to provide all emergency medical care prescribed by a duly licensed physician (M.D.); this care may be given under whatever conditions are necessary to preserve the life, limb or well-being of my dependent.*

As a parent/legal guardian of , I hereby give the staff at VLCA, permission to photograph my child while engaged in indoor and outdoor activities. I understand that these photographs will be used for advertisement of the center or classroom display only and that no monetary compensation will be either received or given.
